Critical Incident Reporting, Correctional Facility Administration - 472-0410-25
Description of Agency
The Michigan Department of Corrections (MDOC) currently oversees 26 correctional facilities throughout the State, and each facility is responsible for critical incident reporting. A critical incident is an event likely to significantly impact individuals, safety, or operations at a correctional facility or attract widespread adverse public attention. Reporting critical incidents ensures MDOC’s chain of command is informed and prepared to respond to inquiries from various stakeholders, such as the Executive Office, Legislature, family members, and media. A post incident review is also completed for certain critical incidents to ensure staff responded as required by MDOC policies and procedures. MDOC’s annual critical incident boilerplate report states 4,471 critical incidents occurred during calendar year 2024.

- To assess the effectiveness of MDOC’s efforts to report all critical incidents occurring at correctional facilities.
